OFFICE VISITS

Each time you visit our office, we will ask for your current insurance card.  It is extremely important that we verify your insurance coverage at each visit.  We will verify your address, home phone number, work phone number, and the reason for your visit.  If this is your first visit to our offices, you will complete a packet of new patient information.  This information is important for the physician to accurately assess your medical condition.  Each year you will be asked to update this information for your medical record.  Included in the paperwork, will be our financial policies.

DIAGNOSTIC

At each of our practices, we have skilled x-ray technologists who will take x-rays deemed necessary for your medical treatment by our physicians.  If you have had x-rays taken or MRI studies, please bring them with you to your visit.
